Fix: repeated installs accumulate .exe files — now cleans up claude.old.exe, claude.*.exe before each install. Only claude.cmd + claude.orig.exe remain (#17)

Uninstall now hints hash -r / restart terminal
PATH hint detects actual shell (bash/zsh/fish) instead of hardcoding ~/.zshrc
Uninstall removes launcher when no backup exists (prevents broken claude command)
which claude detection for pnpm/npm/yarn installs
