Fix: measured cost actually reaches the board
Real-pipeline validation of v2.84.0 caught a bug the fixtures missed: the
SubagentStop hook (and log-verdict.sh) write cost-history.log project-local
(<cwd>/.great_cto/), but readVerdicts enriched only from the global
~/.great_cto/cost-history.log — so measured cost never reached the metrics and
the board stayed on the estimate. readVerdicts now reads the project file first
(global as fallback). Verified end-to-end on real session subagent transcripts:
4 agents → source: 'measured', real $20.38 summed (cache tokens included).
