    rendered, and one evidence cell described a browser action never performed). The report is
    now a fixed ordered template (built / evidence table / guard checks / convergence /
    finding fates / defaults / not-verified / worktree) where an empty slot renders as
    "none - " instead of vanishing, evidence cells must copy from real tool results in
    the session ("did I actually run this?" is the stated test), and a terminal completeness
    pass checks every slot before sending.
