fix(ux/ia): U-5 — Dashboard CTA dedupe (removed duplicate Open-Pipeline header button + duplicate Scan-all-sources Quick-action tile). v1.58.3 QA flagged 4× Pipeline / 4× Scan entry-points on Dashboard. v1.55.5 promoted the two P0 hero CTAs (✨ Auto-pipeline a URL + 🌐 Scan now); the header 📋 Open Pipeline button and the Quick-action 🌐 Scan all sources tile were then strict duplicates (sidebar already routes to /pipeline; hero already routes to /scan). Removed both in public/js/views/dashboard.js. Hero pair + sidebar remain canonical entry-points. 917 → 918 unit. (U-5)

Breaking Changes

Removed header button '📋 Open Pipeline' and Quick-action tile '🌐 Scan all sources' from Dashboard; existing hero CTAs ('✨ Auto-pipeline a URL', '🌐 Scan now') remain.
Updated public/js/views/dashboard.js to eliminate duplicate navigation entry-points.
