feat(ui): UX-A3 (v1.58.55) — Dashboard active-provider chip. Above-the-fold chip in public/js/views/dashboard.js hero now surfaces whether the OR-model fell through to a live provider (e.g. ⚡ Live evals: Anthropic claude-sonnet-4-6) or whether the user is in 📋 Manual prompt mode (no API key set). Re-fetches on providers-changed (dispatched by #/config save) and on visibilitychange (cross-tab refocus). Reuses the existing /api/status/providers endpoint (no new API surface). Two i18n keys × 8 locales (dash.provider.live, dash.provider.manual); one CSS rule (.dash-chip--provider). Lifecycle cleanup on hashchange away from #/dashboard so listeners don't stack across navigation (M-1 discipline). 950 → 951 unit. (UX-A3)
