Improvements

MCP: Configurable session limit per user

The maximum number of concurrent MCP sessions per user can now be controlled via the MCP_MAX_SESSION_PER_USER environment variable. Previously this was hardcoded to 5.

Set it to any positive integer to raise or lower the limit to suit your deployment. If unset or invalid, the default of 5 is used.
