Fix cross-process bootstrap for long-running Copilot CLI sessions

The bootstrap TTL check was comparing against sessionStarted (session creation time) instead of briefGeneratedAt (last activity time). For copilot_code sessions running for more than 10 minutes, this caused every MCP process startup to skip protocol state restoration, blocking iranti_search with attend_required even after a successful iranti_attend(pre-response).

Bumps to 0.3.14.
