Complete README rewrite to reflect the actual system at v1.3.1.

What changed

The old README described v0.6.0 — a basic REST control plane with 45 MCP tools. The real system has grown substantially through Phase 1 and Phase 2. The new README covers:

• Version badge updated to v1.3.1 with MCP tools count (69)
• OS Primitives section — descriptions of all 8 primitives built across v0.7.0–v1.3.1:
  • Event Kernel, Process Signals, VRAM Scheduler, Working Memory, Audit Kernel, Transactions, Agent Lineage, Streaming Task Outputs
• Full API Reference — every endpoint group documented:
  • Task submission (streaming, non-blocking, depends_on, cancellation)
  • Agent lifecycle (spawn, suspend, resume, terminate, handoff)
  • Lineage & call graphs (lineage, subtree, blast-radius, critical-path)
  • Locks, transactions, heap, audit, events, signals, model status
• MCP Tools table — all 69 tools organized by category
• Architecture tree — updated with all new files added through Phase 1 and Phase 2
• Roadmap table — v0.7.0–v1.3.7 with status column
• Testing section — pytest commands per suite (unit, integration, lineage, streaming)
• Hardware section — updated GPU/RAM requirements
