LazyCodex Install Repair and Cleaner Codex Skills

OmO v4.15.1 is a patch release focused on LazyCodex reliability and cleaner Codex skill discovery.

LazyCodex now repairs stale marketplace-managed installs instead of quietly skipping update work when the local cache or managed command links point at deleted payloads. Healthy marketplace installs still stay on the marketplace path, while broken local cache state gets explicit repair behavior.

Codex skill discovery is cleaner. The legacy ultraresearch surface is hidden from shipped Codex payloads, ulw-research remains the canonical research entrypoint, and designpowers reference docs no longer appear as unintended top-level omo: skills.

No breaking changes or migration steps are expected.
