Fixed Consultant NEEDS_PLANNING handoffs creating both a specific
Consultant-authored incident and a second generic runtime incident. Millrace
now validates and adopts the authored incident when valid, and synthesizes a
runtime fallback only when no valid authored incident exists.
Made Consultant handoff registration durable and idempotent across result
replay, daemon restart, incident lifecycle movement, and same-name queue
collisions. Invalid or conflicting incoming artifacts are quarantined with
audit evidence instead of remaining claimable by Auditor.
Made adopted-incident metadata updates atomic and reset registration state on
runtime startup so sequential daemon owners preserve first-registration
authority without creating duplicates.

Both millrace-ai and the optional millrace-web package are versioned and
published as 0.21.1.
